  • (a)

A mover shall not limit valuation on a shipment of household goods to an amount less than sixty cents ($0.60) per pound per article in the shipment. The rejection or selection of additional valuation coverage shall be made in writing on a form prescribed by the Director. The form shall fully advise the shipper of the nature of the limitation of valuation and shall state that the minimum coverage is equal to the limit referred to in subsection (a) unless otherwise requested. The heading of the form shall be in 12-point bold type and shall state: "By signing this form, you are waiving certain valuable coverage which protects your possessions above the minimum amounts set by law.
